MAINTENANCE OF RI REGISTRATION

This will be your only reminder that it is time to submit the information required to permit you to continue as an RI in FY 2003. The following information must be submitted in September for FY 2003 registration, which begins October 1, 2002. For uninterrupted service all required information must be received by October 31, 2002. For timely processing, it is recommended that all mail be sent by a carrier other than the U.S. Postal Service. Please mark "RI RENEWAL" outside of renewal package.

1. Cashiers or certified check for $455 made payable to the Treasurer of the United States, or authorization to charge the renewal fee if you are approved for credit card use,
2. Statement required by 49 CFR Part 592.5(e), (Attachment 1)
3. Current business interest statement, (Enclosure 2)
4. The addresses of all of your facilities: (Enclosure 3)
1. vehicle modification,
2. vehicle storage,
3. record storage, and
4. mailing.
5. Recall Service Insurance Policy for FY 2003 with notarized signatures,
6. Changes in any application information not previously reported.
